The Federal Trade Commission (FTC), Washington, D.C., requires that all funeral homes throughout the United States provide a General Price List to anyone who inquires about funeral arrangements. In compliance with Federal Law, we provide the General Price List to you. The prices listed are subject to change with notice.

The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangement selected will include a charge for our basic services and overhead. If legal or other requirements mean you just buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

Payment Policy


We understand that to lose someone you love is a devastating human experience. It can be traumatic and seriously impact one’s judgment. As payment in full is due before services, it is important to be realistic with respect to funeral expenditures.

We do not want your choices to be more than is affordable nor less than you would like to arrange. What is important is that arrangements be in keeping with your wishes and budget.

We believe in the recognition and meaningful tribute of a life that has been lived and will compassionately assist in arranging an appropriate and significant service to meet your individual needs.

Introduction

Planning a funeral is probably one of the most difficult tasks that one will ever have, albeit one of the most important ones as well. Making arrangements for the appropriate farewell for your loved one is complicated by the need to make decisions quickly and by the emotional realities of the situation. Our goal is to guide you through this important process by truly understanding your wishes and providing you with all the options to assist you in creating a uniquely personal service that respects your preferences and those of your loved one.

This booklet is more than a General Price List. It is also an educational tool, and is designed to provide you with the knowledge required to make informed decisions. Together with the support from our trained and experienced funeral professionals, we trust that your experience, while difficult, will at the same time be rewarding and fulfilling as you create a truly special occasion to be shared by family and friends, and warmly remembered by all.

Department of Human Services
(Medicaid Cremation Services)

Due to Department of Human Services (DHS) strict spending limits on Medicaid recipients, Boies-Ortega Funeral Home will only be able to assist families choosing to use Medicaid death benefits for cremation and memorial services.

Only upon payment in full from the family and/or written verification from DHS of payment forthcoming, may a memorial service or cremation be scheduled with our company. Please note that verifications of payment from DHS can take several weeks and sometimes even longer if the family fails to fully cooperate with DHS.

To qualify for DHS death benefits, your family member must be receiving Medicaid benefits prior to their death. Medicaid funded memorial services and cremations cannot exceed $2,500.00. In most cases for people 60 years old and older. Medicaid will pay up to $1,500.00. The caseworker will examine the assets of the deceased before determining the Medicaid benefit. The existing assets are then deducted from any death benefit that would be paid. For example, if a 67 year old woman passes away and had $600.00 in a savings account, Medicaid will only pay $900.00.

The person who signs the funeral home contract is responsible for the difference between the benefit from Medicaid and the bill. This balance must be paid in FULL at the time of arrangements and prior to any services being rendered.

PLEASE NOTE: Final resting places (e.g. niches, columbaria, mausoleum, crypts, and rose gardens) are exempt from Medicaid spending limits. In other words, an appropriate final resting place and memorialization for a loved one can still be purchased by families utilizing Medicaid benefits and cremation.

Basic Professional Services of the Funeral Director and Staff

Our fee for the services of the funeral director and staff may include, but is not limited to: Staff available 24 hours a day, including weekends and holidays to respond to the initial request for services; consultation with the family or responsible party; preparation and legal filing of necessary State authorizations and permits; notification of Social Security; and following the funeral; assistance in obtaining veterans benefits, fraternal benefits, life insurance benefits, PERA and other work-related benefits. Also included in this fee are accounting and billing costs, regulatory compliance, legal and audit fees, postage, telephones and insurance, and county, state, and federal taxes and fees.

$375 Embalming

Embalming is a process which provides temporary preservation of the body and eliminates health hazards. Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it. Such as refrigeration if the final disposition does not take place within 24 hours of death. Embalming is required if you: 1) Select a service with visitation 2) Arrange for transportation by common carrier or interstate service 3) Select arrangements that require us to hold the remains for more than 24 hours in lieu of refrigeration.

Services of funeral director and staff, and use of equipment in organizing, supervising, and directing funeral services. Includes coordination with those providing portions of the ceremony such as cemetery, minister, music, fraternal organizations, military personnel, pallbearers, and attendants to floral and other tributes to be arranged before the service begins. Our professional and compassionate staff acts as hosts to family and friends attending the service, guiding and supporting bereaved family, greeting guests and ushering, supervising parking, making sure guests sign a memorial book and receive a memorial folder, take memorial contributions, and includes all printing of memorial accessories.

Services Unique to Cremation

When there is not a request for embalming, refrigeration is required by state law 24 hours after death (CRA 12-54-111) until cremation can take place. So that the deceased remains in a refrigerated state, we respectfully request that viewing for family e for no longer than 20 minutes from the time we agree to begin. There is no facility or staff charge for the 20 minute viewing period. There is a fee of $125.00 for preparation of the deceased if it is necessary.
